We are happy to provide these membership applications for your convenience.  Please print out the appropriate application, fill it out entirely, and bring it with you to your local Club between 1pm - 9pm.  Please also be prepared to bring:

¨ Your child's (children's) birth certificate(s)

¨ $10 Application Fee (Cash or Money Order only - no personal checks/credit cards please)

¨ Your tax information OR two recent pay stubs
